• Were you denied unemployment benefits in Massachusetts or New Hampshire?  Why?

• Were you discharged? And if so, what reason did the employer give?

• Did you quit? And if so, why did you quit?

• Why do you believe you are entitled to unemployment benefits?

• What is the date of your hearing (if known)?
